Mosaic Counseling Online Scheduling Terms of Service and Privacy Notice

  1. Acceptance of Terms

By accessing or using Mosaic Counseling’s online scheduling system, you acknowledge that you have read, understood, and agreed to these Terms of Service and Privacy Notice. If you do not agree to these terms, please do not use the online scheduling system.

  1. Purpose of Online Scheduling

Mosaic Counseling provides referral coordination, intake, scheduling, and mental health service support. Submission of an online scheduling request does not establish a therapist-client relationship, guarantee services, or constitute emergency mental health care.

Appointments and referrals are subject to provider availability, clinical appropriateness, insurance verification, and other eligibility requirements.

  1. Eligibility

Online scheduling is available for:

  • Adults age 18 and older.
  • Children accompanied by a parent or legal guardian
  • Minors aged 14 years and older who may independently request and receive outpatient mental health services pursuant to Michigan Compiled Laws § 330.1707.

Special Notice Regarding Minor Consent

Michigan law permits minors age 14 and older to independently consent to outpatient mental health services under specific circumstances.

This provision does not apply to:

  • Prescription of psychotropic medications
  • Inpatient psychiatric hospitalization
  • Pregnancy termination referral services

Services provided under this authority are limited to twelve (12) sessions or four (4) months, whichever occurs first, unless additional authorization is obtained as permitted by law.

  1. Scheduling, Cancellations, and Attendance
  • Appointment requests are subject to provider availability.
  • Clients are encouraged to provide at least 24 hours’ notice when canceling or rescheduling appointments.
  • Repeated no-shows, late cancellations, or misuse of the scheduling system may result in restrictions on future scheduling privileges.
  1. Emergency Services Disclaimer

The online scheduling system is not intended for emergency situations.

If you are experiencing:

  • A mental health crisis
  • Thoughts of harming yourself or others
  • A medical emergency

Call 911, contact the 988 Suicide & Crisis Lifeline, or go to the nearest emergency department immediately.

Mosaic Counseling cannot guarantee immediate response to online submissions.

  1. Electronic Communications

By submitting information through the online scheduling system, you consent to communication by phone, email, text message, secure electronic systems, or other methods reasonably necessary to coordinate services.

While Mosaic Counseling utilizes reasonable safeguards to protect information, electronic communication may involve risks that cannot be eliminated.

  1. Information Accuracy

You agree to provide information that is accurate, complete, and current. Mosaic Counseling is not responsible for delays, scheduling errors, or referral issues resulting from inaccurate or incomplete information provided by users.

  1. Privacy and Confidentiality

Mosaic Counseling is committed to protecting your privacy and maintaining the confidentiality of your information in accordance with HIPAA, the Michigan Mental Health Code, and other applicable laws.

Information We May Collect

  • Name and contact information
  • Demographic information
  • Insurance information
  • Presenting concerns and referral needs
  • Scheduling and service utilization of information
  • Communications related to referrals and services

How Information May Be Used

Your information may be used to:

  • Coordinate referrals and mental health services
  • Match you with appropriate providers
  • Schedule appointments
  • Verify eligibility and insurance information
  • Communication regarding services
  • Meet legal and regulatory requirements

Authorization for Referral Coordination

By submitting information through Mosaic Counseling’s scheduling system, you authorize Mosaic Counseling to use and disclose information you provide for the purpose of coordinating referrals and connecting you with participating providers, practices, or programs that may be appropriate for your care.

Only information that is reasonably necessary to facilitate referral and treatment coordination will be shared.

  1. Your Privacy Rights

You have the right to:

  • Access your information as permitted by law.
  • Request corrections to your information.
  • Request restrictions on certain uses and disclosures.
  • Obtain a copy of Mosaic Counseling’s Notice of Privacy Practices.
  • File a privacy complaint without fear of retaliation.
  1. Confidentiality for Minors

Information provided by minors receiving services under Michigan’s minor consent law will remain confidential except when:

  • Disclosure is authorized by the minor.
  • Disclosure is required by law.
  • There is a substantial risk of harm to the minor or others.
  • Abuse, neglect, exploitation, or another mandatory reporting situation exists.

Mosaic Counseling encourages involvement of a trusted parent, guardian, or supportive adult whenever clinically appropriate and permitted by law.

  1. Technology and System Disclaimer

Mosaic Counseling makes reasonable efforts to maintain secure and reliable online systems. However, we do not guarantee uninterrupted availability of the scheduling platform and are not responsible for:

  • Internet outages
  • Third-party software failures
  • User device malfunctions
  • Unauthorized access beyond reasonable safeguards
  • Delays caused by technical interruptions outside our control
  1. Limitation of Liability

To the fullest extent permitted by law, Mosaic Counseling shall not be liable for indirect, incidental, consequential, or special damage arising from use of the online scheduling system, including delays, scheduling interruptions, or technical failures.

  1. Modifications

Mosaic Counseling reserves the right to modify these Terms of Service and Privacy Notice at any time. Updated versions will be posted on the website. Continued use of the scheduling system constitutes acceptance of any revisions.
